Use of Interreality Care for Severe CHF
Mr. Sanchez has multi-organ system failure and was admitted/readmitted 6 times over 5 months in early 2016. The last time he was readmitted, he had a cardiac arrest when he walked into the ER. Mr. Sanchez had to be intubated and stayed at the ICU/hospital for 15 days. It was estimated he only had 6 months to live. Multiple physician groups recommended end of life hospice or nursing homes.

Clinical Information Platform for Sob and edema
Ms. NB is a 94-year old whose past medical history consists of leukemia, hypertension, coronary disease, and heart failure. In August, her shortness of breath and edema were getting much worse. Her pulse ox would drop from the 90s to low 80s just from walking 10 to 15 ft, making it difficult for her to go out to eat at the diner only 200 ft away from her apartment. In addition, she had an ulcer developing on her left shin. NB's prognosis was extremely poor. The patient and her family were very concerned and hoped to get her through Thanksgiving (same day as her birthday) and Christmas.
To accomplish the task, Duxlink assembled a special team of physicians, technologists, and office coordinators to provide intensive Interreality care with on-site visits and on-line care. After a series of tests and monitoring, an individualized treatment plan was implemented. The collective parts of Duxlink's cloud-based Clinical Information Platform were used to manage the patient's care. The EMR and the online imaging system allowed medical experts to review her patient information remotely. Finally, the monitoring system allowed clinicians to review her submitted weight, blood pressure, pulse ox, and heart rate in real time and make immediate clinical decisions for frequent adjustment and fine tuning.
Over 2-3 weeks of live monitoring and on-time aggressive medical treatments through Duxlink's Clinical Information Platform, NB responded quickly, experiencing significant water loss (~14 lbs), a decrease in shortness of breath, improved lab values, and stabilized blood pressure and heart rate. Since mid-November, Ms. B has been doing very well, her heart failure resolved with evidence of heart failure index, ProBNP levels dropped from 6,000 to <1,500 (normal cut off = <1,800). Her ulcer was healed and she started enjoying walking and dining out as before. She was able to enjoy a big birthday party during Thanksgiving, entertaining over 100 family and friends from all over the US.

I have a history of atrial fibrillation and had an ablation in the past. I always have concerns and sometimes fears if the A Fib is back again when I don’t feel well. Sometimes, I have had to go to the emergency room or doctor’s office at the moment of questioning. This is even harder when I travel either on business trips or vacations with my wife, especially in our motorhome.
